⚠️ Medical Training (Prioritisation) Act 2026 — game-changer
This law introduced priority for UK graduates and ILR/Settled Status holders in selection for Foundation Training (FY1) and Specialty Training. 2026 entry — priority at offer stage; 2027+ — also at shortlisting.
What this means for a refugee doctor without ILR: most cannot realistically enter FY1 directly in 2026-27. The realistic path is:
1. Get GMC registration via PLAB
2. Trust grade post / Clinical Fellow / Medical Support Worker in an NHS Trust (Band 5-6 £29-46k)
3. Build up UK clinical experience for 1-3 years
4. Once you receive ILR (after 5 years on refugee leave + an ILR application) — you become eligible for Foundation/Specialty with priority

This changes the old recommendation of "PLAB → FY1". The new one is "PLAB → Trust grade → ILR → Specialty".

Step-by-step pathway

STEP 1 · ENIC + IELTS/OET (3-6 months)
UK ENIC Statement of Comparability £59.40 (charity rate £25.60 for refugees via RefuAid). IELTS Academic 7.5 overall + 7.0 in each domain (~£200) OR OET Medicine Grade B (~£470). Specialised medical English courses are often free for refugee doctors via REACHE / Building Bridges.
STEP 2 · PLAB 1 (3-6 months prep)
Computer-based MCQ. £283 (refugees: first 2 attempts FREE). Multiple sittings per year in the UK plus international centres. Pass rate ~70% first-attempt. Free prep via REACHE / Building Bridges. Plab1Keys / Pastest materials (~£300-500).
STEP 3 · PLAB 2 (6-9 months prep)
OSCE clinical examination. £1,036 (refugees: first 2 attempts half-price = £518). Manchester only (GMC clinical assessment centre). Pass rate ~80% first-attempt. REACHE provides PLAB 2 prep + clinical placements in NHS hospitals for refugees.
STEP 4 · GMC REGISTRATION (1-2 months)
Registration application £433 (effective April 2026). Primary source verification via MyIntealth (replaced EPIC 2024-25) — FREE for refugees. Certificate of Good Standing alternative evidence is accepted for refugees from war-zone countries (sworn affidavit, employer references).
STEP 5 · TRUST GRADE / FOUNDATION (after ILR)
Refugee without ILR: Trust grade post / Clinical Fellow / Medical Support Worker (Band 5-6 £29-46k). Build up UK clinical experience.
Once ILR is granted (5+ years): Foundation Programme (FY1 £38,831; FY2 £42,008) → Specialty Training (ST1 £52,656 → ST8) over 3-8 years. Pay rise +3.5% April 2026.

Band 2-3 £24,465-£26,598. 6-week training course. Hospital + community. A good fit for those with a medical background.
🩺 Medical Support Worker (MSW)
Band 6 £38,682-£46,580. For GMC-registered international doctors waiting to build a CV for Foundation/Trust posts. The main category after MTPA 2026.
🏥 Trust grade / Clinical Fellow
After GMC registration. Not on a formal training pathway, but clinical work in an NHS Trust. £35-55k typical. A stepping stone to Specialty Training after ILR.

🏛️ BMA Refugee Doctor Initiative
~900 supported. Free BMA membership, BMJ Learning access, BMJ subscription, BMA library, 24-hour counselling service. Critical for wellbeing during a stressful pathway.
💷 RefuAid Equal Access Loan
£500-£10,000 interest-free, character-based. Healthcare workers = 50%+ of all loans. <1% default rate. Evaluated by LSE in 2025.

🧠 Mental health — name it honestly, do not ignore it
The PLAB pathway after displacement / war / persecution is a heavy load. PTSD, depression, sleep problems, the after-effects of sexual trauma — these are real barriers. Helen Bamber Foundation and Freedom from Torture can provide medical reports that the GMC accepts as mitigating circumstances for extra exam time, alternative format, or a re-attempt after a fail.
